COVID-Related Forms

Please fill out the forms (below) if you or your child:

• Were in close contact with someone who has COVID-19

• Have COVID-like symptoms or tested positive for COVID-19

• Tested negative for COVID-19

We ask families and staff to fill out these forms whenever they experience any of these situations. The forms are available here:

• Negative COVID-19 Test Results or Alternative Diagnosis Form

Travel Guidance

If possible, avoid travel outside our local DC, Maryland and Virginia area. However, if you do go somewhere, the guidance upon returning to DC is to:

1. Limit daily activities until being tested for COVID-19 and getting a negative result. DC advises the test be done 3 to 5 days within 72 hours of coming back.

2. Once you receive your test results, please upload them to the Negative COVID-19 Test Results or Alternative Diagnosis Form.

Required Travel Survey

If you plan to travel, please do both of the following at least two weeks before you leave or as soon as you know your travel plans:

1. Complete the Yu Ying Travel Questionnaire.

2. Staff email our Human Resources team at HR@washingtonyuying.org.
